Report a Code of Student Conduct Violation

Code of Student Conduct violations are behaviors that include, but are not limited to, alcohol, disorderly conduct, drugs, hazing, harassment, sexual misconduct,* physical violence, or other violations of university policies, regulations, or rules.

To report non-academic misconduct, please use our online reporting form.

*Please note that there are varying ways to report sexual misconduct. For more information about your reporting options, please go to: Sexual Violence Awareness

We also encourage you to contact our confidential resource or Victim Advocate at 508-999-4584 or bvieira@umassd.edu to discuss your options in a private and confidential setting.

Report Concerning, Worrisome, or Threatening Behaviors (CARE Network)

Faculty, staff, students, and guests are encouraged to use the online referral form to report behavior that they perceive as concerning, worrisome, or threatening. Please submit a report.

Report an Academic Integrity Violation

Academic violations are behaviors that include, but are not limited to, cheating, plagiarism or falsifying official records related to the University. General classroom misbehavior is considered a Code of Student Conduct Violation (see above report form).

To report academic misconduct, please use the report form.

Appeal of a Conduct Decision

You have three (3) business days from the date the decision letter to submit your appeal. Please submit your appeal here.

The Sexual Assault Anonymous Reporting Form is intended for the anonymous and confidential reporting of rape and sexual assault—it is NOT a police report.

Reason for Absence Form

The Absence from Class for Emergencies or Health Reasons policy explains that class attendance rules and handling of absences are matters to be resolved between individual faculty members and students. The Division of Student Affairs can assist a student with communicating with faculty when the student is unable to do so. Fill out the form.
